A lost document affidavit is a sworn statement that a specific document is genuinely lost, so a duplicate can be issued. For some documents that is all you need; for others, the issuing authority also insists on a police complaint first — and coming to us without knowing which applies just means an extra trip.

When a police complaint is also needed
For high-value or identity-critical documents — a passport especially — the issuing authority typically wants a police complaint or FIR alongside the affidavit, not instead of it. For many everyday documents (a lost mark sheet, for instance), the affidavit is usually sufficient on its own.
